* To qualify for the student registration fee, you must provide proof of fulltime enrollment at a recognised university or college.

PRE-CONFERENCE SESSIONS (Thursday 22 November)

Pre-conference sessions are at an additional cost to conference registration. Places at these sessions are strictly limited and bookings are essential through the online registration process.

Smart casual attire is recommended for all conference sessions.

REGISTRATION TERMS AND CONDITIONS

PAYMENT OF REGISTRATION FEES

All accounts must be paid in full prior to the Conference. Early Bird Registration fee will be available until Friday 21 September 2018. Payment for Early Bird Registrations MUST be received by COB on 21 September 2018. After this date the Early Bird rate expires and the Standard rate will be applied automatically to all unpaid registrations. All accounts must be paid in full prior to the Conference. The following payment methods are accepted:

Credit Card

The conference accepts MasterCard and Visa. Payments will appear on your statement as ‘The Society of Hospital Pharmacists of Australia’. Credit card payments will be processed via a third party gateway and attract a 1.61% surcharge. AMEX is not accepted.

Electronic Funds Transfer

Bank details will be on your invoice, or contact us via email to mm2018@kaigi.com.au. Please include your surname and invoice number as reference when transferring funds. Email your remittance advice to Kaigi Conferencing and Events (mm2018@kaigi.com.au).

CANCELLATION OF REGISTRATION

We hope that you will not have to cancel your registration, however, should this be necessary cancellations must be notified in writing to the Conference Secretariat. Cancellations received prior to 21 September 2018 will receive a full refund less $150 (inc GST) administration charge. Refunds for cancellations received on/after 22 September 2018 will only be made in exceptional circumstances. If you are unable to attend, a substitute is welcome at no extra charge. Please advise the Conference Secretariat of the details of the substitute delegate in writing.

CONFIRMATION OF REGISTRATION/TAX INVOICE

Once you have completed the online registration form you will receive a confirmation email from info the Conference Secretariat. Please note that the email confirmation will be sent to the email address entered when registering for the conference. This confirmation email will include your tax invoice as an attachment.

PRIVACY

Information collected as part of the conference registration process is owned by the Conference Organising Committee and Kaigi Conferencing and Events. We only access information that you voluntarily give us. We will not provide this information to any third party, other than as necessary to fulfil your request (such as to accommodation providers).

INSURANCE AND LIABILITY

It is recommended that participants obtain adequate cover for travel, health and accident insurance before they depart for the conference as organisers cannot accept responsibility for personal injuries, or loss of, or damage to, private property belonging to the conference participants and accompanying persons.

PHOTOGRAPHY

Photographs may be taken during the conference and reproduced for SHPA promotional purposes.
